Physical Exams

Physical Exams consist of 2 Parts:
Part 1 is appointment based.  Appointments are available Monday, Wednesday, or Friday, 10 a.m. and 10: 30 a.m. for brief to be scheduled through the appointment line.  Service member must have a memo approved by their commander unless they have orders in hand.

• Part 1: report to Physical Exams located at Conner Army Medical Home (CAMH), P10506A, Euphrates River Valley Dr.  Follow instructions for lab work and x-rays when required.
• Part 2 is a scheduled appointment with your provider that can be scheduled at same time as part 1.

Flight Physicals

• Soldiers currently assigned to the Falcon Clinic for medical care, will contact the Falcon Army Medical Home, Building P2065, Wheeler Sack Airfield, to schedule flight physicals. Please call 315-774-1451 for more information.                                                      
• Soldiers not assigned to Fort Drum will contact the Aviation Troop Medical Clinic, Building P2065, Wheeler Sack Airfield, to schedule flight physicals. Please call 315-774-1451 for more information.
• Soldiers not assigned to Fort Drum will contact 315-774-1117.

 
Note: Separation Physicals must be done within 180 days of terminal leave in order to be processed in a timely manner.
