Command Sergeant Major

Command Sgt. Maj. David C. Cook

CSM Cook enlisted as a Combat Medic in the U.S. Army Reserve on 14 June 2004, in Denver, Colorado. He attended Basic Training at Fort Jackson, South Carolina and Advanced Individual Training at Fort Sam Houston, Texas. Upon graduation, he was assigned to the 5502nd U.S. Army Hospital, Denver, Colorado. In March 2006 CSM Cook enlisted in the active U.S. Army and was assigned to the 984th Military Police Company at Fort Carson, Colorado.

He is currently assigned as the Command Sergeant Major for the U.S. Army Medical Department Activity, Fort Drum, NY. His previous assignments include—10th Mountain Division (LI) Chief Medical NCO Fort Drum, New York, 44th Medical Brigade Headquarters and Headquarters Company First Sergeant, Fort Bragg, North Carolina; Security Force Assistance Command, Headquarters and Headquarters Company First Sergeant, Fort Bragg, North Carolina; 4th Cavalry Brigade Medical Operations NCO and Ambulance NCO OC/T, Fort Knox, Kentucky; 529th Military Police Company Platoon Sergeant, Wiesbaden, Germany; U.S. Army Health Clinic Schweinfurt Operations NCO, Schweinfurt, Germany, and Warrior Transition Unit Cadre, Fort Gordon, Georgia—He has also served one tour in Iraq and one tour in Afghanistan with the 984th Military Police Company, and was deployed with the 10th Mountain Division (LI) as the Joint Task Force Southern Border Senior Enlisted Medical Advisor.

His military education includes the Warrior Leaders Course, Advanced Leaders Course, Senior Leaders Course, Master Leaders Course, Unit Prevention Leaders Course, Equal Opportunity Leaders Course, Sexual Harassment/Assault Response Program Victim Advocate Course, Master Resiliency Trainer Course, Master Fitness Course, OC/T Academy, Army Instructor Course, and Battle Staff Course. He graduated from the Sergeants Major Academy (Class 73) and holds a Bachelor of Arts in Leadership and Workforce Development from the U.S. Army Command and General Staff College. He is currently in pursuit of a Master of Business Administration—Healthcare Administration from the University of Central Missouri.

His awards and decorations, include the Bronze Star Medal, Defense Meritorious Service Medal, Meritorious Service Medal (4OLC), Army Commendation Medal (6OLC), Army Achievement Medal (5OLC), Navy Achievement Medal, Good Conduct Medal (6th Award), National Defense Service Medal, Armed Forces Service Medal (1 Bronze Star), Afghanistan Campaign Medal (2 Bronze Stars), Iraq Campaign Medal (2 Bronze Stars), Global War on Terror Service Medal, NCO Professional Development Ribbon (with Numeral 5), Army Service Ribbon, Overseas Service Ribbon (with Numeral 3), NATO Medal, Valorous Unit Award, Meritorious Unit Citation (2OLC), Master Combat Medical Badge, Master Combat Action Badge, Expert Field Medical Badge, Combat Medical Badge, and Combat Action Badge. He is also a recipient of the Sergeant Audie Murphy Award, the Order of Military Medical Merit, the German Schützenschnur (Silver), and the German Armed Forces Proficiency Badge (Gold).
